Clinical Supervision Individual Hour (1hour) $45

Insurance
We will file insurance claims with your insurance carrier for you, as long as we accept your insurance.
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Reduced Fee
Reduced fee services are available on a limited basis.
Payment Accepted
Cash, check and major credit cards (Visa, Mastercard and Discover)
Payment expected upon delivery of services.
Cancellation Policy
Please call as soon as possible if you know you will not be able to make your scheduled appointment time. If you do not show up for your scheduled therapy appointment, and you have not notified us , you will be required to pay $25.00 of the missed session.
